Step 1

Choose a method for enrollment
(SSN and date of birth needed to enroll
spouse and/or children)

Step 2 

  • LIVE enrollment by telephone

            Call 501-943-4182 (Enrollment Center)

            9 am - 5 pm Monday - Friday

EMPLOYEE BENEFITS FREQUENTLY ASKED QUESTIONS

Do I need additional information to enroll my spouse and/or child(ren)? 
Yes. You will need a social security number and date of birth to add dependent(s) 

What if I miss the deadline to enroll in employee benefits? 
You may make changes at our next annual Open Enrollment effective April 1st. Or, if you have a
Qualifying Life Event (marriage, divorce, birth, etc.) you have (30) days to contact HR for enrollment instructions.


When does the medical insurance annual deductible start over?

April 1st of each year 

When does the dental insurance annual deductible start over?
January 1st of each year

What is the deductible for the Vision Plan?
The Vision plan does not have an annual deductible.

When will I receive insurance cards?
Within 7-10 days of your insurance effective date.

What if I lose my insurance card?
